Just improvise during the intro. I think Thurston uses a slide.
When the noise dies down, start droning the low E:

D----------------------------                
A----------------------------               
E--0-0-0-0-0-0-0-0-0-0-0-0-0- etc

then:

G------------------------------------
D------------------------------------
A------------------------------------
E--3-3-3-3-3-3-3-3-3-1-1-1-1-1-1-1-1- slide

just do a random slide way up the neck at the end of each repetition.

back to randomness during outro 'chorus'..... drone the low E again
for the outro.
