first of all, with all due respect, as a serious psychiatrist this woman shouldn't be revealing details to non-colleagues.
some people have obsessive compulsive disorder that causes them to have intrusive upsetting thoughts of sexual nature, but they don't get turned on by them. these thoughts are disturbing to them, so they are the ones that most commonly search for professional help.
in many other cases, people have all sorts of sexual fantasies that may or may not be carried on, between adults, in role play terms where nobody gets hurt.
as for peadophiles and rapists, they tend of be sociopathic individuals that are uncapable to control these urges for long. What turns them on is the idea of having power and/or hurting another. they are normally "respectable normal" people with children and family. i don't think it's possible for sociopaths with that kind of tendency to control themselves, and they don't normally look for help either.
